ProCare clinical director Dr John Cameron said he was aware of a growing trend of health data, which could have its pros and cons. "You can use that data for a whole range of different things, and really, if you're getting down into that highly granular perspective of your own health, you're probably 99 per cent of the way there anyway." On the downside, there was the risk of reading too far into anomalies in the data or self-diagnosing - and apps could not replace the GP. "We are trying to take that helicopter view and find out if the patient is heading in the right direction, rather than focusing on those minuscule changes."

CARE PROJECT

ProCare has six Waitemata practices involved as intervention practices with this pilot providing a comprehensive assessment, care planning and additional support to frailer older people at risk of admission to hospital or needing residential care. The CARE (Co-ordinated care, Assessment, Rehabilitation and Education) Project was launched in September and is already proving its value in addressing the growing needs of our ageing population. Our district has the highest population of senior citizens, so the project has enormous importance. The CARE Project is dedicated to coordinating and enhancing the care of people aged 75 and over and Maori and Pacific people aged 65 plus. The general practices in the first stage of the project are funded to give longer appointments with enrolled patients. This provides time to do a comprehensive assessment of a patient’s needs and create a specific care plan which aims to keep them comfortable in their own home and avoid unnecessary admissions to hospital.

Losing weight with the family (Auckland City Harbour News)

Family is at the centre of a weight loss challenge in Auckland. The Aiga Challenge aims to increase the health of the Pacific community and decrease the obesity level. The eight-week family programme involves 42 Pacific Island churches enrolled in the Auckland District Health Board's Healthy Village Action Programme. Pro Care is one of the health care providers that has teamed up with the ADHB.

20 ProCare practices assessment and Foundation certified

Between 1 July 2015 and 30 June 2016 20 ProCare practices were Foundation certified. The Foundation Standard represents what is considered to be the legal, professional, and regulatory requirements for general practice. Foundation Standard is included as the entry level criterion for general practices in the Ministry of Health’s Integrated Performance Incentive Framework (IPIF) and in the PHO Services Agreement. Practices must meet the Foundation Standard by no later than 1 July 2017. The programme requires practices to provide evidence of meeting standards set by the College, followed by a reassessment cycle every three years.

Big four’s solution to burden of unmet mental health need (NZ Doctor)

The four largest primary care networks want a better deal for patients bringing mental health and addiction problems into the surgery. General practice feels unsup­ported and is not coping well with this burden of disease, says Allan Moffitt, clinical di­rector at the Auckland-based network ProCare. Dr Moffitt is keen to see new, nationally consistent approach­es so that when people present “we can do something with it that is not just sympathetic noises”.

Pegasus joins ProCare in baulking at costs to GPs of enrolment service (NZ Doctor)

Pegasus has joined ProCare in sitting tight on the rollout of the National Enrolment Service, saying practices should not have to pay the cost of implementation and maintenance. The costs to general practice, being charged by PMS vendor Medtech, are to be discussed at the PSAAP (PHO Services Agreement Amendment Protocol) meeting in Wellington tomorrow. Last week, ProCare advised its customers not to activate the system, which will cost practices $675 to sign up and then $100 monthly.

Accreditation process stalled by cost woes for hard working practice (NZ Doctor)

An Auckland solo GP and a prac­tice manager feel their practice is in limbo after completing the RNZCGP’s new entry-level standard processes. The duo spent eight months revising existing systems and instituting new ones in line with the college’s Foundation Standard. Practice manager Sha­ron Dorsett says the job was fin­ished to PHO ProCare’s satisfac­tion in mid-December 2015.
